Appointments
If you live in the Seattle area: Please call us at (206) 215-2480. We will work with you and your local doctor to make preparations for your visit.
If you live outside of Seattle: Without leaving home, you can send us your medical records and we will provide you with a preliminary assessment of your eligibility for seed implantation. We can then help you arrange a convenient time for a comprehensive evaluation at our office in Seattle. Click for details.

For your convenience, you can fill out our New Patient Registration Form at home and bring it with you to your first appointment.
To view the forms listed below, you will need Adobe Reader. You may download Adobe Reader for free at www.adobe.com. Please bring your completed forms with you to our office at the time of your visit.
Preliminary Assessment
In order to help us determine if seed implantation is an appropriate treatment option for you, we ask that you send us the documents listed below by mail or by fax. When your records have been reviewed by an SPI physician, we will call you with the results of our assessment and discuss the desirability of your coming to Seattle for a comprehensive evaluation. There is no charge for this service and we are generally able to get back to patients 3-5 business days after receiving a full set of records. - A completed Int. Prostate Symptom Score Sheet
- Biopsy reports (If a consultation visit is scheduled in Seattle, we usually ask you to have your pathology slides sent to us for review.)
- PSA blood rest results (the official laboratory reports)
- Your physician's notes regarding your cancer
- Reports from any x-rays and bone/CT/MRI scans
- Reports from prostate ultrasound examinations
- A copy of your insurance card(s) (both sides)
IMPORTANT: We will be best able to review your records and respond promptly if you can gather these seven items together and mail or fax them at the same time. This way, you can be confident that we have all of your records and will not be kept waiting in the event that one or more items were not forwarded from a hospital or doctor's office.
