If you Live in the Seattle Area:  Please call us at (206) 215-2480. We will work with you and your local doctor to make preparations for your visit. 

If You Live Outside of Seattle: Without leaving home, you can send us your medical records and we will provide you with a preliminary assessment of your eligibility for seed implantation. We can then help you arrange a convenient time for a comprehensive evaluation at our office in Seattle. Click here for more details.

NOTE: For your convenience, you can fill out our New Patient Registration Form at home and bring it with you to your first appointment.
